Know Before You Go
The 8 Steps of the Procedure
A detailed look at everything that happens on surgery day — so nothing catches you off guard.
Hair line design, anaesthesia, follicle extraction and implantation are the 4 steps that always take place. Depending on the chosen technique, there are potentially 8 steps in total.
🔬
STEP 1
Consultation & Hairline Design
The doctor determines the cause of hair loss and confirms suitability for a transplant. They then calculate:
—How many grafts can be harvested from the donor area
—How many grafts are needed for full coverage
✂️
STEP 2
Shaving & Preparation
Right before the operation, the back of the head is shaved to 1–2 mm. The shaving is typically a thick line at the back — the hair won't be fully trimmed.
💉
STEP 3
Sedation & Local Anaesthesia
Surgery begins with sedation via vascular access followed by local anaesthesia on donor and recipient areas. Additional anaesthesia is injected at the inner brow corners to ease headache during recovery.
Not general anaesthesia
Lidocaine (C₁₄H₂₂N₂O) is most commonly used. Alternatives include Prilocaine and Markain.
🌾
STEP 4
Hair Grafting
Grafts are extracted with a micro-motor using a hollow metal pipe of 0.5–1.0 mm diameter. The device rotates and cuts through the skin, removing not just the hair bulb but the entire follicular unit with surrounding tissue.
❄️
STEP 5
Graft Preservation
Harvested grafts are cleaned and stored in petri dishes kept at 0–4°C. They are separated by size — from single-hair to four-hair grafts — ready to be loaded into DHI pens.
💎
STEP 6
Opening of Micro-channels (FUE only)
Micro-cuts are created with a sapphire blade in the recipient area. These are shallow and tiny in diameter. The angle and direction of each channel are critical — mimicking your hair's natural growth direction ensures an undetectable result.
🖊️
STEP 7
Graft Implantation
Grafts are inserted one by one with DHI Choi pens. Each pen can be loaded twice per use, with the team reloading simultaneously throughout the operation.

STEP 8
Post-Surgery
After surgery you'll be fully awake and mobile. You may feel tired, slightly confused, or have a mild headache. Go directly home or to your hotel, eat something, and rest as much as possible. Feeling dizzy and tired is completely normal.

⚠️ Heads Up
Heads up — something to expect soon
Around Day 14, many patients notice hair shedding from the transplanted area. This is called shock loss — and it is completely normal and expected. The follicles are not affected. This is simply the old hair shafts cycling out to make way for new growth.

What you might read online — vs. reality
5 common myths · tap to expand
▾
"My grafts are falling out after surgery"
✓ It's the hair shaft shedding, not the follicle. The follicle stays put. Growth resumes from Month 3.
"No growth at 3 months — transplant failed"
✓ Dormant phase is completely normal through Month 5. First growth typically appears Month 4–6.
"My hairline looks uneven / asymmetrical"
✓ Asymmetry at 3 months is swelling and dormancy — not the final result. Final shape visible at 12 months.
"Shock loss means poor graft survival"
✓ Shock loss has no correlation with graft survival. It's a normal growth cycle reset — expected and temporary.
"Results depend on the surgeon, not aftercare"
✓ Compliance with your aftercare protocol has a measurable impact on density outcomes. Your daily effort counts.

✓ Day 4 — Shock Loss is Normal
You may notice some transplanted hairs starting to fall out. This is completely expected — it's called "shock loss" and it's part of the healing process. The follicles remain healthy underneath and will regrow stronger from Month 3.

💜
Finasteride 1mg
Once daily · Every morning · Starting Day 20 · Indefinite
⚠️ Most Important Medication Post-Transplant
Non-adherence to finasteride is the #1 reason patients are disappointed at Year 2. Skipping doses allows DHT to keep thinning your native hair around the transplant.
How It Works
🧬
Blocks DHT
Finasteride inhibits the enzyme that converts testosterone to DHT — the hormone responsible for male pattern hair loss.
🛡️
Protects Native Hair
Transplanted grafts are DHT-resistant. Finasteride protects the native hair around them, maintaining the full result.
📊
Clinical Evidence
Patients with 90%+ adherence retain 31% more native hair at 12 months vs. those who skip doses.
Schedule
⏰
When to take
Every morning, same time. With or without food. Set a daily alarm — it takes 30 seconds.
🗓️
How long
Indefinitely. Stopping finasteride reverses its protective effect within 6–12 months.
